The common thread is all of these issues is that there are no meaningful management metrics about these many awful situations. Without management metrics the people who have the most responsibility for allowing these situation to exist are never held to account. Journalists also have a role to play, but they need the right sort of information in order to do their job in an efficient effective way. Journalism is a very important part of a liberal democracy, but is less fit-for-purpose in the very fast moving news landscape that exists today than it has been in the distant past.
